The CHNA process is an opportunity for us to engage with patients, providers, and partners to deeply understand your communities’ health needs resources and work collectively to respond. Therefore, we ask for less than 5  minutes of your time to complete a survey to guide and help us identify, understand, and respond to your communities’ health needs.
